Acute Home Support

Under the Acute Home Support contract, Pohlen provides assistance with showering toileting, dressing, cleaning, cooking and other domestic support to patients within 24 hours of discharge from a stay at Waikato Hospital. This is a short term service only. Referral is made by the discharging ward or the district nursing service. No nursing care is provided by Pohlen as this is done by the District Nursing Service.

ACC Home Based Rehabilitation - non serious injury

ACC Home based Rehabilitation is similar to acute home support but adds nursing care as well. The service is funded by ACC and will extend for a longer period than Acute Home Support. The service period varies from patient to patient dependent on individual circumstances.

Private Home Care

Anyone in the community can contract to have a Pohlen Home carer assist with personal care (showering, toileting, bathing, dressing, hair care etc) or home care (tidying, cleaning, laundry, basic food preparation).
