Hospital and Rest Home Care

Pohlen has a reputation for providing high quality, hospital level continuing care for older members of the community.

Whilst we are not a Rest Home, we maintain our certification to provide Rest Home care in order that we can care for couples with different levels of assessment or for those people being assessed for the first time and who may wish to transfer to a Rest Home after assessment.

Assessments are performed by Disability Support Link (DSL), who are the Needs Assessment and Service Co-Ordination Agency for the Waikato.

Transitional Care

Transitional care is provided to patients over 65 years old to allow an early discharge from an acute ward at Waikato Hospital following a severe medical or orthopaedic event. Admission to the service is via assessment and referral from the Waikato Hospital Transitional Care Team. The service is designed to facilitate the transition from hospital to home and/or from medical dependence to functional independence.

Pallative Care

Palliative care is provided during the final weeks of life. The emphasis in nursing for palliative care is on the dignity and comfort of the patient. Patients will normally have a room to themselves that is big enough to accommodate visitors for extended periods, even overnight stays. Family discussions can be held in the quiet room next door Referral to the service requires assessment and approval by DSL.

Rehabilitation and Exercise Area

Pohlen opened its outdoor rehabilitation and exercise facility in March 2010. This facility is free to use (at your own risk), and is designed for those requiring gentle exercise using only their own body weight or for use in conjunction with a physiotherapist to re-establish mobility after a stroke or leg injury.
